Situation

In the Czech Republic, there is growing pressure on home care and support due to an ageing population, shorter hospital stays and a lack of follow-up care. However, there is no uniform system for planning, patient categorisation and continuity between health and social services. This contrasts with Switzerland, which, thanks to a well-developed system of non-hospital care, has excellent home care and support. Consequently, the KOPEC – Komplexní domácí péče (HOPE – Holistic Perspective in Home Care) development project is engaging in cooperation with Bern University of Applied Sciences to adopt best practices from Switzerland. The project aims to develop and pilot a functional model for a reference network for home care, based on standardisation, methodological support, digital tools and capacity building. The target groups are home care providers, nursing staff (particularly in home care), informal carers (family members), patients or clients (indirectly), and experts in education and methodology.

Course of action

Modelled on the Swiss nursing home care system, a reference network comprising at least 48 home care providers is being established. These providers will receive methodological support. In addition, a training programme for professionals is being developed. This includes courses on case management, telemedicine, quality assurance and psychosocial support in home care. A support programme for informal carers will also be established, comprising training sessions, educational materials, instructional videos and online resources. Furthermore, in the spirit of forward-looking care management, assistive technologies, telemedicine and electronic documentation will be integrated and piloted. As part of the cooperation with Bern University of Applied Sciences, online counselling, online continuing education and three internships in Switzerland are being offered to familiarise participants with the Spitex system and other home care and support services.
